Start with any other weapon (because the perk works for the 1st attack after you swap to sword), swap to the sword, use heavy attack, press and hold jump+shatter button together but with jump being a frame (5-20ms) earlier. Triple jump and Stomp-EE5 are going to make you go much further. The process is the same as Warlock. First, you need to jump, then light attack, jump again, and Shatterdive.
